   ปานสีน้ำตาล Café-au-lait macules (CALMs) เป็นปานแต่กำเนิดลักษณะปื้นราบสีน้ำตาลอ่อน ขอบเขตชัดเจน มักปรากฏตั้งแต่วัยทารกและขยายขนาดตามตัว โดยทั่วไปไม่อันตราย แต่หากมีขนาดใหญ่หรือจำนวนมากอาจเกี่ยวข้องกับโรคทางพันธุกรรม การรักษาหลักใช้เลเซอร์ความยาวคลื่น 532 นาโนเมตร เพื่อทำลายเม็ดสี ซึ่งควรปรึกษาแพทย์ผิวหนังเพื่อวางแผนการรักษา 

ปานน้ำตาล (Café au lait)  เป็นปานราบสีน้ำตาลอ่อน ปรากฏตั้งแต่แรกเกิด ไม่สามารถหายได้เอง ควรพบแพทย์ผู้เชี่ยวชาญด้านผิวหนังและเลเซอร์เพื่อวางแผนการรักษา เนื่องจากปานชนิดนี้ให้ผลการรักษาที่ไม่แน่นอน

รายละเอียดปานสีน้ำตาล
* ลักษณะ: ปื้นราบสีน้ำตาลอ่อนหรือกาแฟใส่นม ขอบเขตค่อนข้างชัดเจน
* การเกิด: ปรากฏตั้งแต่แรกเกิด หรือภายใน 2-3 เดือนแรก
* การขยายขนาด: จะขยายขนาดเพิ่มขึ้นตามการเจริญเติบโตของร่างกาย
* ความเสี่ยง: ส่วนใหญ่ไม่มีอันตราย แต่หากพบปานสีน้ำตาลขนาดใหญ่หรือมีหลายจุด (6 จุดขึ้นไป) อาจบ่งชี้ถึงโรคทางพันธุกรรม เช่น โรคท้าวแสนปม (Neurofibromatosis)
* การรักษา: ตอบสนองต่อการรักษาด้วยเลเซอร์ Qs หรือ Picosecond Laser หรือเลเซอร์ความยาวคลื่น 532 nm อย่างไรก็ตาม ปานชนิดนี้อาจมีโอกาสกลับมาเป็นซ้ำได้

เป็นปานราบสีน้ำตาลอ่อน คล้ายกาแฟใส่นม
เห็นเป็นขอบเขตชัด มีสีค่อนข้างเสมอกันทั้งแผ่น
ปรากฏตั้งแต่แรกเกิด ไม่สามารถหายได้เอง มักจะมีขนาดโตตามตัว
ส่วนใหญ่ไม่พบความผิดปกติอื่นร่วมด้วย

การรักษา
ปัจจุบันมีการนำเลเซอร์ระบบ Q-switched และ Picosecond มาใช้รักษา
แต่ให้ผลการรักษาที่ไม่แน่นอน บางรายได้ผลดี บางรายเข้มขึ้น
ส่วนใหญ่รักษาไม่หายขาด พบปานกลับมาเกิดซ้ำได้บ่อย และบริเวณที่รักษาด้วยเลเซอร์อาจมีสีผิวกระดำกระด่างชั่วคราว
•ตัวอย่างที่หมอใช้คือเทคนิคดูแลปานน้ำตาลด้วยโปรแกรมพิโคแบบแบ่งส่วน Fractional 532 nm Picosecond Laser for Brown Birthmark Treatment 
• โดยอาจใช้เทคนิคทำบริเวณเล็กๆก่อนเพื่อดูการตอบสนองครับ (test spot)

ข้อควรระวัง
หากมีปานชนิดนี้ขนาดใหญ่และมีจำนวนมาก (> 0.5 ซม.ในเด็ก หรือ > 1.5 ซม.ในผู้ใหญ่ และมีจำนวนตั้งแต่ 6 ตำแหน่งขึ้นไป) อาจพบร่วมกับความผิดปกติอื่นของโรคทางพันธุกรรม (เช่น โรคท้าวแสนปม ฯลฯ) ร่วมด้วย ควรจะได้รับการตรวจวินิจฉัยเพิ่มเติมจากแพทย์

หากต้องการทดลองรักษาด้วยเลเซอร์ แนะนำให้ปรึกษาแพทย์ผู้เชี่ยวชาญด้านเลเซอร์ผิวหนัง โดยทดลองรักษาเป็นบริเวณเล็กๆ ก่อน เพื่อประเมินการตอบสนองและผลข้างเคียงหลังการรักษา

* ปานโอตะ (Nevus of Ota): ปานสีน้ำตาลเข้ม เขียว หรือน้ำเงินเทา มักขึ้นเป็นปื้นบริเวณใบหน้า รอบดวงตา

* ปานดำลึก (Congenital Melanocytic Nevus): ปานสีน้ำตาลเข้มหรือดำ อาจมีขนขึ้น และนูนกว่าผิวหนังปกติ
การดูแล: ควรพบแพทย์เพื่อวินิจฉัยแยกโรคและตรวจสอบขนาดหรือการเปลี่ยนแปลงของปานอย่างต่อเนื่อง

What Are Café-au-lait Macules and How Should They Be Managed?

Café-au-lait macules (CALMs) are congenital pigmented lesions characterized by flat, light-brown patches with well-defined borders. They often appear in infancy and gradually enlarge in proportion to body growth.

In most cases, they are benign and not harmful. However, the presence of large lesions or multiple spots may be associated with underlying genetic conditions.

The primary treatment modality involves laser therapy—particularly 532-nm wavelength lasers, Q-switched lasers, or picosecond lasers—targeting melanin pigment. Consultation with a dermatologist is essential to develop an individualized treatment plan.

Clinical Characteristics of Café-au-lait Macules
• Appearance: Flat, light-brown patches resembling “coffee with milk,” with relatively well-demarcated borders
• Onset: Present at birth or within the first 2–3 months of life
• Growth: Gradually enlarge in proportion to the patient’s growth
• Risk: Generally benign; however, ≥6 lesions may suggest genetic disorders such as Neurofibromatosis type 1 (NF1)
• Treatment response: Variable; lesions may respond to Q-switched, picosecond, or 532-nm lasers, but recurrence is common

Treatment of Café-au-lait Macules
Café-au-lait macules are permanent lesions and do not resolve spontaneously.

Laser treatments such as Q-switched and picosecond lasers are commonly used; however, outcomes are unpredictable:
• Some patients show significant improvement
• Others may experience darkening or minimal response
• Recurrence is frequent
• Temporary post-inflammatory pigmentary changes may occur

An example approach includes:
Fractional 532-nm picosecond laser for brown birthmark treatment, which may enhance pigment clearance while minimizing adverse effects.
Before initiating laser treatment:
• Perform a test spot in a small area
• Assess treatment response and potential side effects

Precautions and When to Investigate Further

If lesions are:
• Larger than 0.5 cm in children or 1.5 cm in adults, and
• Present in ≥6 locations,

they may be associated with genetic disorders (e.g., Neurofibromatosis). Further evaluation by a physician is recommended.

Other Pigmented Lesions That May Mimic Café-au-lait Macules
• Nevus of Ota: Dark brown, bluish, or grayish patches, typically on the face around the eyes
• Congenital Melanocytic Nevus: Dark brown to black lesions, often raised and may contain hair

Management: Dermatologic evaluation is necessary for accurate diagnosis, monitoring of lesion changes, and appropriate treatment planning.
